New Patient Registration
We are currently operating a 'balanced list'. This means that in any month we can register the same number of patients that left the practice the previous month. Using this method means that that our list neither gets larger nor reduces in size. This will be reviewed in March. If we are able to accept your request we welcome new patients who live within our practice area.
As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are required to attend a health check with one of our Health Care Assistants. Please also read our 'Patient Charter' under policies on the top black bar.
Medical treatment is available from the date of registration. Please contact reception for further information.
You may print off a registration form, fill it out and bring it in with you on your first visit to the practice if you wish but please note WE CANNOT guarantee registration if our monthly quota has already been filled. You will be required to show two pieces of ideantification at registration, one photo ID and one as proof of address, eg bank statement or utility bill.
