New Patient Registration
Our list is currently open and we welcome new patients who live within our practice area.
As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are required to attend a health check with one of our Health Care Assistants. Please also read our 'Patient Charter' under policies on the top black bar.
Medical treatment is available from the date of registration. Please contact reception for further information.
You may print off a registration form, fill it out and bring it in with you together with proof of identification as detailed below, on your first visit to the practice if you wish. Please note WE CANNOT guarantee registration until we have established that you live within our practice area. It would be helpful with the registration process and with tracing any previous records if you can bring two pieces of identification, preferably one photo ID and one as proof of address, eg bank statement or utility bill.
